Connie Raffa and Rachel Hold-Weiss to Speak at the National Association for Home Care & Hospice 30th Annual Meeting and Exposition
Connie Raffa and Rachel Hold-Weiss will be speaking on “Choosing a Good Death: Palliative Care, the Original Accountable Care Organizations” and will address the differences between hospice and palliative care programs (PCP) and reasons for creating a PCP. This presentation will also explain the similarities between PCP programs and ACOs, medical homes and IAH and describe design options for a PCP organization, as well as identify state and federal regulations, fraud and abuse issues and compliance requirements when creating a PCP.
Connie Raffa and Rachel Hold-Weiss will also present on “The Hospice/Nursing Home Partnership in Caring for the Terminally Ill” and will discuss the barriers to collaboration between hospices and SNF. This presentation will also address the revised hospice COPs and proposed SNF COPs impacting hospice care for nursing home residents and inpatient hospice agreements, as well as identify hospice/SNF compliance issues.
